Questionire for students. parents and teachers! Please help out!

I have to do a research paper for my sociology class and my topic is american social problems with education. Here is a little "interview" I came up with just to get some general ideas. I do have to cite sources so please don't respond if you don't feel comfortable giving a few personal information like: name, email and such.
