Exclamation Transfer of ownership with APHA & PtHA. Help!?

I need to get my mare put in my name. I'm terrible, horrible. It's been over a year since I finished paying for her and I should have done this ages ago. Ricci's papers say to send in a blank along with the certificate, but where do I get the blank? What's the transfer fee? I can't find anything out online, but I'm kind of a dolt about stuff like that. It seems to be the same thing with PtHA. Do I give the date I bought her [010609] or today's date? I want to get this mailed in yesterday, so your help is greatly appreciated!!

For APHA the seller should have signed and filled out the area under seller, and you need to put your info under buyer, then send in the certificate w/ the transfer fee and membership fees if you are not yet a member (and I don't know what they are, sorry). And you'll put in the date of you bought her. That should be all you need to do, don't know what a blank is. Don't know about PtHA.

Fees are listed on the transfer form.
Transfer fee is $15 but it requires you to be a member, the one year fee for membership is $40 it appears.

If I can find it, anyone can. As Speedracer so nicely told the whole world, Google hates me. I so know what it is like to not be able to find something that everyone else can find.
