fetlock injury

my 18 yr old thoroughbred had an injury about 1 month ago. we aren't sure as yet when he did it we think it was either in the horse float or when i did a 1 rein halt on the morning before pony club. anyway he ihas done something to his fetlock we think the vet has had a look and origiinally said a tendon then a ligament then soft tissue and now we aren't sure. it doesnt seem to be getting better he'll get better then worse then better then worse. his fetlock is a little bit swollen and he is quite warm he is bandaged, hosed and given bute every second day and he's just not recovering quickly any ideas what it mayb be? i have thought about sesamoiditis but have decided it is unlikely ut stil possible. any help very very very appreciated thanks.
