Not necessarily. All three of my horses had it last winter, not one of them had bald patches from scratching and rubbing.

The tell tale sign of lice is usually excessive scratching and rubbing itchy spots. Parting the hair and finding lice is also a good way ;)

If it does turn out to be lice and you're using powder, I highly recommend you wear a mask, it's awful to inhale!

I checked her and did find two lice unfortunately so I assume there's more :( they were on her back, I didn't
See any around the bald spot.
I will be buying powder first thing tomorrow.
She has a really thick winter coat any ideas on how to get it deep to her skin?
I'll invest in a mask as well!

Part the hair, rub it deep in. Do that all the way down the spine from head to tail. Don't worry about the rest of her body - if you get the spine you'll get them all. She's probably rubbing at the chest because she can itch that bit easily rather than because that's where they are hanging out.
