Unhappy Help): Need to know whats wrong with horse!

I have a horse, appendix paint gelding that is ill I've called the vet but she is very busy.
He has like Mud on his lips but when I touch it he pants in pain and I tried warshing it but it will not come off. He is also very lethargic but I know he's not colicing he pooped alot when I was at the barn. Also he is extremely scratched up- not the usual clip or two he gets (i treated them)
There is no toxic plants there and only foxes are there. I just want to know whats wrong with him ):

How is he moving? Sounds like he might be in pain from falling - but without more info that's a complete guess.

Will he eat? Offer him some food and see if he will eat.

Is his breathing faster than normal? Do you know the signs of colic?

Is he in pasture or stall? If he is in a pasture you might wanna see if there is any clues to what he might have hurt himself on. What you see ..the "mud" is probably a dark scrab with dirt..
