Need Sheath Experience/Help

My gelding dropped today while I was grooming him, and it was actually one of the only times I've ever seen him drop, ever.
Point being.
I cleaned his sheath about a month ago (I only clean it about every year or so to remove the bean etc) but never actually have had him drop while cleaning it, so imagine my surprise when I noticed some orange nodules along with the regular rigamarole.
Has anyone ever noticed this before with their geldings? Should I be concerned? He doesn't seem to be in any discomfort from it.
Help? Advice?
