Sheath cleaning?

How often should this be done? My vet comes twice a year (if everyone is healthy). She sedates and then power floats teeth, while they are loopy she does a sheath cleaning. Is it necessary to do this twice a year or will once suffice?

Every horse is different. Some horses get dirtier faster.
