sheath problem

I have a 24-ish year old appy gelding. When he urinates he doesn't drop down, and never has. The Doc said it was because he wasn't able to get it out because the opening was really small. His sheath wouldn't get too bad, it was somewhat dirty but nothing major. Well this winter his sheath area got infected,and really swollen, this had never happend in the 8 years we have had him. So we gave him anti biotics for it. That seemed to work. And the Doc came out to take a look at his penis, he said it looked ok. Now there is a different problem, he has major creamy white discharge, coming from his sheath, there always seems to be quite a bit right at the opening. Does anyone know what this could be?
