My horse got something that looks similar but on his nose... The skin cracked and peeled and he lost all the hair. It started out as an allergy and then sun made it worse, resulting in chronic dermatitis. We have to keep it constantly covered and zinced. We treated it with a cream that was antibiotic, insect repellant and sunscreen, and now just keep it covered.

Just a thought.

Also it could be something related to/similar to greasy heel?

That black spot looks like scar tissue. Is the horse out in a field?

Is there any history of trauma to those legs in those areas? I would talk to the vet about the possibility of sequestrums--damaged bone or other tissue from a previous traumatic accident that triggers chronic inflammation in the area. Sequestrums cause inflammation and swelling and then a draining tract to the outside of the body, then they close back over and start the cycle again.

If it IS a sequestrum, you need to get it checked out immediately. They require surgery. My horse had one as a three year old but after surgery she came back with no complications at all
