Really Bad Jumping Form?

Uh, haha. My horse's form is horrible. I've only plopped him over 4 little jumps (if they're considered that) before, but how do you get him to do a nice bascule? Can someone tell me really good jumping exercises to help our form? (Mine is bad too!) Like, tell me how far apart to place them and what fun exercises I can do with him? I showed a video that shows you how bad we both are. He like, doesn't jump it and knocks them down. He's only 5, used to race, so he's still pretty young and inexperienced, just like me. So, since i'm admitting to not being a very good rider, please don't tell me I suck. Lol Just help us both get better.
