Unhappy First lease how do I do it right?

I am leasing my horse of 5 years to a good friend who has had horses and knows how to take care of horses, but I have never leased a horse out and I am afraid if I do it wrong I may make a mistake. I am keeping the horse at my house until her place is finished and ready for horses, but she is going to be paying all food, vet, shoeing, and extra bills of horse. We are having her sign a contract for the lease so there is no mis confusions and she is signing a realse form to ride him on our property without having reliabilty on us. we are using an online preintable form so I think thst will work but need to ask to make sure. also she is going to be moving him to her house in about two months and at that time I am going to ok the location before hand but if place is not acceptible how do I deal with it? also we are doing a 6 month lease on him to make sure she is the right home and at the end we are going to decide if I want to give him to her permenatly is that a good amount of time to see how she takes care of him? also she has said that she is going to use the same vet, get special shoeing and keep everything the same so I am feeling more confortable about that. THE ONE MAIN PROBLEM IS that the horse is unsound, and she knows and has all the info on whats wrong with him but I am wondering what kind of contract to write out to make sure she understands because every now and then he will trip or fall while being riden and if she were to fall I wouldnt want to be sued for having that happpen so how do I deal with that? If anyone has any extra tips or suggestions please include with reponse it would be greatly appreciated! also Thank you!!

i am just setting up my fifth lease, i bought two of the horses I leased and am pretty sure I will end up buying this one too, just want to make sure we are right for her and she is right for us.
I too used a lease agreement I downloaded off the net, we lined out parts we did not like and wrote in things we wanted.
some things you may want to add or consider:

1) my lease spelled out the location horse was to be kept at and could not be moved to a new facility without approval
2) spell out if you want them to obtain mortality insurance on the horse
3) who will pay for routine vaccines, shoeing etc AND how often those are required to be done..the third horse we leased when owner got her back from previous people leasing her she had not had shots/been wormed or trimmed.
4) spell out who can ride your horse...we wanted my immediate family including kids to be able to ride and had to sign a release.
5) spell out feed, supplements etc..can she be trailered anywhere, can she be entered into shows, gymkhana etc...

pretty much try to spell out anything applicable so it doesn't cause problems later.

Don't take life so seriously. After all no one gets out alive.
