Jumping help needed!

Hi everyone, I am having some some issues with my position that need to be fixed! So first problem is that I think I get too ahead of the horse when I jump which makes her ping over the jumps and makes me get jumped out of the saddle. Also, I don't grip at all with my lower leg when jumping, so my lower leg and heel are loose.
So I know what things I need to work on, but when I get to the jump its like I forget everything. Does anybody have suggestions for remembering what my form should be like at the jump?? I know that riding a lot in two point would help my leg but I unfortunately only get to ride once a week...

My trainer used to have us put one dollar bills under our legs, so wherever your leg hits the bottom of the saddle flap, but a dollar bill there and just walk/trot/canter around and see how long you can hold it there. It drastically helped my form over jumpers/equitation in general. It is hard to train yourself if your only riding once a week, but do this for about 10-15 minutes as your warm up, should help(:
