I have an old horse with missing teeth and he would definitely choke on a large chunk of carrot, shredded is the only way he gets them these days.
If you have a horse with good teeth that doesn't tend to bolt it's food then feeding them larger pieces of carrots is probably fine.
A horse usually doesn't die from choke because they can still breathe with something lodged in their throat. It can give them pneumonia if they aspirate food & saliva into their lungs or I suppose if the owner didn't get help for the horse it would eventually die from dehydration because typically they will not eat or drink while choking.
