Starting to feel a little hopeless

Hi all. So I'm just starting to get really hopeless with my horse. He is 17.2 hand ottb, 15 years old. He limped most of the summer and just recently after lots of poultice, cold hosing, stall rest and bute is looking and doing better. He even trotted into the barn the other day. I was so happy. Then tonight he trotted in the pasture after I turned him out and then stopped and limped a few times and then was fine. But he wouldn't trot anymore. He is also looking on the thin side and has like no topline. I don't want to hop on and ride him and make things worse if something is wrong but I just don't know what to do anymore.

Vet? Chiro?

Is it a hoof? A mechanical alignment (skelatal)? A specific injury?

What is his current caloric intake and use?

What has your farrier and vet said? Has he seen a chiro?

How long have you had him? How long has he had these problems?
