My TB did this to me when his saddle was too far back, and I'm talking maybe an inch to an inch and a half. It had scooted back but the girth was in the same place, so I thought it was just my eyes.
Check how your saddle fits and where you're putting it, that would have been my first thing to look at, even before checking the bit.
