Last January I made basically this exact same post, but the spot came back, and I am desperately trying to figure out what it is.

So, Jan. 2019, my mare Pepper developed this on the left side of her withers:

This summer, she has slowly developed this in the middle of her back:

I'm calling the vet asap but last year they tested ringworm and rain rot and found nothing. So, I'm at a loss of what to do. They gave me antibacterial/antifungal shampoo to wash her with and it really seem to affect it last year. It did eventually go away.

When I first saw this spot, it was tiny and I didn't think much of it, but I decided to start washing it with the soap just in case. It only got worse since.

Like I said, I'm planning on calling the vet ASAP but nobody seems to have ever seen this before and I'm really worried.

It's extremely itchy and may be painful.

If anyone has any advice on what it is or how to get rid of it, please let me know. Thanks!

Possibly a fistula, irritated by tack pressure and rubbing..
Get your vet out and ask about it...they can be a nuisance and need some serious intervention of it is this so it not worsen. Each time it reappears it gets more intense and more difficult to clear up.
